The Medical Team

Our goal is NOT to end life , but to make the end of life as full and meaningful as possible. We work to help the patient and family recognize and come to terms with issues of grief and loss and to transform these final times into a beautiful process. It is a journey through pain, but it can be a journey rich with meaning and joy. And that is why we are here.

The Hospice Interdisciplinary Team consist of Administrator- Medical Director-Director of Clinical Services-Nursing Staff- Home Health Care Aids- Social Workers-Chaplains-Volunteer Coordinator-Volunteers-Bereavement Coordinator We provide patient centered care, put others before ourselves, advocate for the wishes of each patient, make time to spend with family, caregivers, facility staff, and other Residents, be dedicated to serving others, be willing to listen and educate and YOU are now the most important part of the team!
