Who We Are

Hospice is specialized care that focuses on maintaining comfort while maximizing quality of life. Hospice provides pain and symptom management services for those with a life-limiting illness. Support is also provided for their families and loved ones during this difficult time. Hospice is a way to continue life’s journey with the best quality of life and the best care possible. When in hospice care: The patient’s physical, emotional, social, and spiritual needs are addressed Family, loved ones, and caregivers are included in care and care planning Care can take place wherever the patient calls home Discomfort and adverse symptoms will be addressed to ensure the patient is as comfortable as possible THE MEDICAL TEAM HOSPICE DIFFERENCE Every hospice organization is regulated to provide the elements of hospice care as dictated by the Centers for Medicare and Medicaid Services (CMS). How that care is provided is what makes THE MEDICAL TEAM Hospice stand apart from the rest. We are dedicated to providing the patient and their family with care that is individualized to their wants and needs. Our goal is to ensure our patients have the best quality of life in their final moments.

What We Do

The Medical Team (Hospice) goal is NOT to end life, but to make the end of life as full and meaningful as possible. Our society tends to run from death and make it a horrifying experience. Death is one of those four letter words. We work to help the patient and family recognize and come to terms with issues of grief and loss and to transform these final times into a beautiful process. It is a journey through pain, but it can be a journey rich with meaning and joy. And that is why we are here. We do not hasten death, but we do allow the person the dignity to die without painful, debilitating treatments that rob the person and their families of meaningful interactions because the person is experiencing painful side effects from treatments. We want life to be lived to the fullest until the final breath is taken. And even after death of one of our patients, we continue to follow-up with the family for the next 13 months through our bereavement program.
